As we age the skin and soft tissues of our face undergo a host of changes. These range from textural changes such as fine lines and wrinkles to more dramatic changes due to gravity such as excessive skin laxity and sagging. This ultimately results in reduced facial definition as volume shifts down from the cheeks, an appearance of heavy jowls and the presence of redundant skin beneath the chin and neck. These changes along with those occurring in our forehead and eyes all contribute to an older, tired and worn out look. Changes in facial volume and bony remodelling also occur however the effects of gravity on our face causing redundant and unfavourably positioned soft tissue cannot be understated.
The facelift is a procedure which was developed to address these changes occurring in the face. Older facelift techniques tightened the face by pulling on the skin and unsurprisingly caused excessive and unnatural tension or a wind-swept appearance. To avoid this, newer and more advanced facelift techniques such as the Deep Plane Facelift have emerged. This technique allows for the repositioning of the soft tissues of the face and neck back to their youthful position by mobilising and tightening a layer of tissue beneath the skin known as the SMAS (superficial muscular aponeurotic system). This layer when tightened moves the skin with it, without transferring any tension to the surface. This avoids the overtight appearance which can be seen in some facelifts and results in a durable, youthful and most importantly natural outcome.
It is common to ask when the best time to have a facelift might be. The progression of facial ageing occurs differently in each individual and this can be compounded by environmental factors such as sun damage and smoking. Patients with less advanced ageing will typically see more subtle results whilst those with advanced facial ageing will feel that the changes are more dramatic. In each case, the aim is to return to the face to its optimum state.
